Reviews of process safety incidents and near misses provide valuable learning opportunities. Papers dealing with incidents, near misses, and the lessons learned are requested. Emphasis should be placed on sharing key lessons learned from the past incidents. In addition, papers on accident scenario analysis, accident consequence modeling and accident frequency estimation will be also accepted in this session.
